What conditions are most commonly managed with RPM?

RPM is most effective for conditions with daily physiological variability: hypertension (blood pressure monitoring), heart failure (daily weight and SpO2), COPD (spirometry and pulse oximetry), Type 1 and Type 2 diabetes (continuous glucose monitoring), and post-acute recovery following hospitalization or surgery. Vervelo supports all of these use cases with condition-specific device configurations and alert protocols.

What are the CMS billing requirements for RPM?

CMS requires: (1) the patient must have a chronic condition; (2) the device must be FDA-cleared and transmit physiological data; (3) the patient must transmit data on 16 or more days per calendar month to bill 99454; and (4) a qualified healthcare professional must spend at least 20 minutes per month reviewing data and communicating with the patient to bill 99457. Vervelo automates tracking of all four requirements with real-time dashboards and alerts.

Can RPM be billed simultaneously with CCM or TCM?

Yes. RPM can be billed in the same month as CCM (99490–99491) and APCM (G0556–G0558), provided the time is not double-counted. RPM can also be billed alongside TCM during the post-discharge monitoring period. Vervelo's unified billing dashboard handles program overlap automatically — flagging time that needs to be separated between programs and preventing double-billing errors.

Who can perform RPM services — does it require a physician?

RPM monitoring and management time (99457 and 99458) can be performed by clinical staff — RNs, MAs, and other qualified personnel — under the general supervision of the billing physician or qualified non-physician practitioner. The billing provider does not need to personally review all data. Vervelo's platform supports multi-staff time tracking with supervisor oversight dashboards to maintain compliance.
